By using the telehealth services offered by Win From Within Wellness (“WFWW”), you acknowledge and agree to the following terms. Telehealth includes video visits, phone consultations, secure messaging, and the remote review of labs, supplements, and wellness protocols.

1. Nature of Telehealth Services

Telehealth allows you to receive functional medicine and wellness support remotely. Services may include: Functional medicine consultations, Nutrition and lifestyle guidance, Review of labs and wellness plans, Education on supplements, medications and peptides, Legally compliant prescribing when appropriate. Telehealth may not be identical to in‑person care and may have limitations.

2. Scope of WFWW Services

WFWW provides: Functional medicine and wellness consultations, Nutrition, lifestyle, and supplement guidance, Education on peptides and other therapies, Prescribing of certain therapies when clinically appropriate. WFWW does not: Diagnose medical conditions, Provide emergency medical care, Replace your primary care provider or Guarantee outcomes or specific results. You agree to maintain a relationship with a licensed medical provider for ongoing medical care.

3. Peptide‑Related Telehealth Disclosures

WFWW may prescribe certain medications and peptides that are legally compounded by FDA‑registered 503A or 503B compounding pharmacies when medically appropriate and within the provider’s scope of practice. When it comes to prescribing, you acknowledge that shared decision making between the patient and the provider will be utilized. You understand that compounded peptides may be used as on or off label meds. You acknowledge that all peptide‑related discussions provided by WFWW do not replace the medical advice of your primary care provider or specialist. You are responsible for discussing any prescribed or recommended peptide therapy with your primary care provider, especially if you take medications, have chronic conditions, or experience side effects. WFWW does not guarantee outcomes or specific results from peptide use.

4. Risks & Limitations of Telehealth

Telehealth may involve risks, including: Technology failures or interruptions, Limited ability to perform physical assessments, Delays in evaluation due to incomplete information and Potential privacy vulnerabilities despite reasonable safeguards. WFWW uses secure platforms but cannot guarantee protection against all unauthorized access.

5. Privacy & Confidentiality

WFWW follows applicable privacy laws, including Illinois privacy requirements. You are responsible for ensuring your own privacy during telehealth sessions (e.g., using a private space and secure device). Telehealth sessions may not be recorded by either party without written consent.WFWW protects your health information in accordance with HIPAA and Illinois privacy laws. Your rights and WFWW’s responsibilities are outlined in the WFWW HIPAA Notice of Privacy Practices. 

6. Patient Responsibilities

By using WFWW telehealth services, you agree to:

• Provide accurate and complete health information

• Use a secure internet connection and device

• Be physically located in a state where your WFWW provider is authorized to practice

• Follow all treatment recommendations as instructed

• Seek immediate medical care for urgent or emergency concerns

• Inform WFWW of any changes in your health status

7. Emergencies

WFWW does not provide emergency medical services. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.

8. Consent to Telehealth

By continuing to use WFWW telehealth services, you acknowledge that:

• You understand the nature, risks, and benefits of telehealth

• You consent to receive services via telehealth from WFWW

• You may withdraw consent at any time by notifying WFWW in writing

• Withdrawal does not affect care already provided.
